Student Shadow (4th-8th Grade)
Please review the following items listed as they must be uploaded before your Admission Application can be processed. Once you have insured you have these items please continue with online registration below.
  • Application for Admission
  • Birth Certificate (issued by County – not hospital certificate)
  • Social Security card
  • Sacrament Certificates (Baptism, Reconciliation, First Communion – if applicable)
  • Recommendation form – Students applying for Grades 1-8, to be completed online
  • School Health Form –
    • Part 1 to be completed by your physician by June 1, see below
    • Part 2 to be completed by parent online
  • Registration Fee ($200 per child – non-refundable).

Health Form
All students applying for Early Fives or Kindergarten will participate in Kindergarten screening.  Students applying for Grades 1-8 will be tested for placement purposes before admission approval.  All students are expected to meet Incarnation School’s standards for academics for placement in the requested grade level.  Acceptance will also be dependent on the student’s behavioral record and work ethic. A representative from Incarnation will reach out to schedule the screening after your admission application has been received.

Our Early Childhood programs conduct general screening in early August in order to assess a baseline for all children entering the program. Information about these screenings will be emailed out in early summer.

After the online application has been submitted and your screening has been completed Incarnation School will review your application for admission. In the spring Incarnation will email out acceptance letters to all families for the upcoming school year. If you are put on a waitlist or if we need to receive more information before a decision can be made, a representative will contact you via email.
Save the Dates:
  • January 22nd from 5-7:00PM is our Winter Open House for grades Preschool /. All interested families are invited to attend whether you are new to the school or just interested in information. Come tour the school, get a bite of pizza, and visit with all our wonderful teacher and staff!
  • November 15 application for enrollment opens to new families and siblings. The items listed in the next tab must be submitted before your Admission Application can be processed. Documents may by securely uploaded through the website or copies may be delivered to the main office.
  • Screening for Kindergarten and Early Childhood programs will take place in late February. Information will be sent out to families as we approach that date.
  • Screening for other grades will also be required. Families who have submitted applications for new students in grades one through eight will be contacted to schedule a placement test. ​
